Without sharing details

You can finish the quiz and read all results without an account or email address. Answers stay in the open page’s memory; refreshing or closing it clears them. They are not saved to your clinic quiz record.

If you request an email

With your explicit consent, your name, email, answers and any suitability information are processed to create and deliver your requested profile. My website hosting and managed email provider process this information for delivery. Email content includes your profile, not the raw medical checklist. Email providers retain delivery information under their own service policies; clinic deletion cannot remove copies already delivered to your inbox.

If you allow me to keep your profile

A separate, optional explicit consent permits me to keep your contact details, answers (including any health-related or pregnancy information), calculated profile, consent choices, date and notice version. Only authorised clinic staff can access these records. I use them to review the concerns you chose to share, not to diagnose you or automatically decide which treatments you can have.

Retention & deletion

Clinic quiz records expire after 90 days by default. I can set retention to 30, 90, 180 or 365 days; a shorter policy shortens existing records too, and increasing it never extends existing records. Expired records are hidden immediately and deleted by a daily task. Security rate-limit records use hashed identifiers and are cleared after a day. Backups may retain deleted data for the hosting provider’s backup period.

Marketing is a separate choice

The marketing checkbox starts unchecked. Receiving your profile never subscribes you to marketing. This website records a preference only; it does not send marketing campaigns. You can withdraw that preference at any time.

Your rights

Email info@leedslaser.co.uk to request access, correction or deletion, or withdraw consent. I may need to verify your identity. You can also complain to the UK Information Commissioner’s Office. Withdrawing consent does not affect processing that already happened.
